2010 W-2 EMPLOYEE WAGE STATEMENTS

Your W-2's were sent to the store in which you are employed on Jan 14, 2012.
FORMER EMPLOYEES
Your W-2's were mailed on January 14, 2012 to the permanent address you provided upon termination of employment. If you did not provide a permanent address, it was mailed to the address printed on your last paycheck. If you do not receive your W-2, please e-mail us at koppebridge@suddenlink.net with your current address and a duplicate will be issued within 10 days. It is your responsibility to provide your permanent mailing address or any address change that is effective at year end. Failure to do so will delay delivery of your W-2. If you don't have e-mail, you may send a self-addressed stamped envelope to:
